BC Team Athlete Declaration Form

 

Welcome Athletes!

All athletes wishing to be considered for a Provincial Team (BC Team Program) must declare themselves for selection using this BC Team Athlete Online Declaration Form.  As per the BC Athletics Privacy Policy, by submitting this form, you are consenting to the use of your information by BC Athletics to determine eligibility for selection, and athlete tracking purposes.  Upon selection to the team, your information may be shared with third parties such as the Canadian Sport Institute (CSI), PacificSport, BC Government Sport Branch, and other provincial and national sport agencies as deemed necessary.  For further information, please read the complete BC Athletics Privacy Policy.

Athlete Declaration Process (2 steps)

1. BC Athlete Declaration Coach Sign-Off.  In an effort to ensure that athletes, coaches, parents and clubs (if applicable) are aware of an athlete's declaration as it pertains to his/her competition plan, BC Athletics will be asking athletes to submit this form with their declaration.  Please read the instructions regarding ways to submit this form.
2. Complete and submit the BC Team Athlete Declaration (below). You can save this form at any time by clicking the save link at the bottom of the form.
